AgentForms is provided by Allowance Inc. By creating, publishing, or submitting a form, you agree to use the service lawfully and to provide accurate information.

Prohibited data

Do not use AgentForms to collect passwords, authentication codes, API keys, payment-card details, bank credentials, Social Security numbers, regulated health information, malware, or illegal content.

Your forms and responses

Creators are responsible for their form instructions, requested data, privacy notices, and treatment of responses. Respondents are responsible for reviewing agent-prepared answers before submission.

Service limits

We may enforce documented limits, suspend abuse, and change or discontinue the service. The service is provided as available without warranties to the maximum extent permitted by law.
